To Request Confidential Data Forms

The following three documents are for use in requesting confidential data from MISO or the Market Monitor.
Please note that copies of these forms should be sent directly to the Midwest ISO to the attention of Wayne Schug and Steve Kozey.  No FERC filing is required.

# 1.  Certification Form - this form will need to be completed FIRST by your agency's general counsel.  Part of this document is a schedule of persons authorized by their commission to  request confidential information from MISO and the market
monitor.  An Electronic version of this schedule is at the back of the non-disclosure form if that is an easier form to use.

# 2.  Non-Disclosure Agreement - Secondly, individuals authorized to request information must sign the non-disclosure agreement. "Attachment EE" is a version whose format allows you to fill in the blanks electronically and submit the form on-line.

# 3.  Information Request Form - Third, you may submit requests.  MISO has offered the "Information Request Form" as a way to do this.
